Nowadays, HIV/AIDS Prevention and Control projects play a very important role in the war of against HIV/AIDS crisis. It is the critical responsibility for project Monitoring and Evaluation (M&E) to find and solve problems on the process of program design and implementation. Meanwhile to make a objective judgment about whether those projects have achieved original effectiveness and summarize the experiences on the process of project implementation, allocate resources of projects effectively, maximize the effect of project, and direct those similar projects how to be carried out. Based on studying the theory and method of Theory-Driven Evaluation introduced by Professor Huey-Tsyh Chen, Public Health school of University of Alabama Birmingham, the paper uses its rational and the M&E System on the process of a HIV/AIDS Prevention and Control project, which is implemented by Marie Stopes International (MSI), funded by Asian Development Bank. While using the theory on the real field site, we got some experiences on the credibility, practicability and limitation of the evaluation theory, and expect to sum up a effective evaluation and assessment tool on HIV/AJIDS projects in order to provide technical support for the evaluation of HIV/AIDS projects.The result shows: (1)the theory-Driven theory includes action model and change model, which provides a clear direction for the practical evaluation and easy to handle; (2)concentrate on main points of evaluation while using the theory;(3) make out indicators of HIV/AIDS project assessment and evaluation scientifically and reasonably under the direction of the theory;(4) the theory empower evaluator to focus on critical point and make a objective assessment on project effect;(5) the theory emphasis on using different approaches and strategies on different evaluation stages.Meanwhile, there are some issues that should be focused on: (1)Scientific and stakeholder credibility, namely, achieve a balance of both kinds of credibility, to pursue stakeholder credibility in the earliest phases of evaluation design but to yield to scientific principles later in the process; (2)use Theory-Driven evaluation flexibly after catching the
