Objective:We measured endothelin-1 (ET-1) and Fast Insulin(FINS) in patients with Acute Ischemic Cerebrovascular Disease(AICVD),To investigate the relationship between the severity of the disease and the levels of ET-1 and Insulin Resistance(IR),the relevance of IR and ET-1,the clinical and Pathophysiology implication of IR and ET-1 in acut cerebral infarction,Aims to help reduce the incidence of ischemic cerebrovascular diseases, and the treatment of ischemic cerebrovascular disease has guiding significance.Methods:Select 100 cases of acute ischemic cerebrovascular disease as a team (including cerebral infarction group of 58 cases, transient ischemic attack group 42 cases), at the same selected patients hospitalized with acute cerebrovascular or healthy charge 82 patients for the control group. All subjects in hospital for five days (except emergency response), fasting 12h, fasting blood collected in the morning with determination ET-1 (radioimmunoassay), fasting blood sugar (glucose oxidase method,FBG)、FINS (radioimmunoassay)。Calculation ISI,ISI = ln 1/(FBG×FINS), Insulin Sensitivity Index after determination, which is Used to denote the body’s resistance to insulin levels。ISI more Lower that says the more serious insulin resistance。Simultaneously recording research object of gender, age, smoking history, weight, blood pressure, Lipid, uric acid as indexes. By measuring data to mean±Standard deviation(X±sd) said, by between groups with independent t test, data of the single factor analysis and factor analysis and unconditional Logistic regression analysis; Variables and the relationship between variables with linear correlation analysis. P < 0.05 for difference was statistically significant difference (p < 0.01 significant statistical significance. Use of SPSS statistical software, 14. 0.Results:1.The single factor analysis showed that the AICVD group FINS, TG, FPG, LDL - C, EF - 1, TC significant higher than those in the control group, the ISI, hdl-c significantly lower than that in control group, P < 0.01 or P < 0.05.2. Unconditional Logistic regression showed that ET - 1, FPG, FINS, ISI, TC and TG, LDL - C, hdl-c enter regression equation, which EF - 1, FINS, ISI, TC, hdl-c is acute ischemic cerebrovascular disease risk factors.3. ET - 1 in CI group, significantly higher than in ATCI group of TIA than LCI group, P < 0.01 or P < 0.05; ISI group in CI group below TIA ATCI group in significantly below, LCI group, P < 0.01 or P < 0.05.4.Double variables linear correlation analysis showed that acute ischemic cerebrovascular disease EF - 1 and the ISI correlation coefficient r = - 0.58, P﹥0.05.Conclusion:1. Acute ischemic cerebrovascular disease body exist on the higher ET- 1 level, existence of insulin resistance.2. ET - 1, IR participated in the acute ischemic cerebrovascular disease pathophysiological events, is acute ischemic cerebrovascular disease risk factors.3. IR is brain size of vascular lesions common risk factors.4. ET - 1, ISI in acute ischemic cerebrovascular disease body, suggesting no significant correlation between the role in cerebrovascular disease and related mechanism may be different.
