Diagnosis and Surgical Treatment of Insulinoma in 23 Cases

Objective To summarize explore the diagnosis and surgical treatment of insulinoma . Methods A retrospective analysis of hospital clinical data from 1998 to 2005 , 23 patients with insulinoma . Results of the group of 19 patients with typical Whipple triad . B ultrasound , CT, the positive rate of diagnosis of selective arteriography in the preoperative were 47.8% ( 11 / 23 ) , 64.7% ( 11 / 17 ), 85.7% ( 6 / 7 ) . With simple tumor enucleation in 14 cases, 6 cases of partial resection , pancreatic body and tail resection , pancreatoduodenectomy cases , laparoscopic enucleation of insulinoma patients . Postoperative blood glucose back to normal , but two cases of pancreatic fistula complications occurred pseudocyst , 2 patients with postoperative symptoms of psychiatric symptoms , no improvement . Conclusion Whipple triad and IRI / G> 0.3 for insulinoma diagnostic significance . B ultrasound, CT, preoperative selective angiography is used to define a clear positioning . Simple tumor excision is the most common surgical method , intraoperative ultrasound and blood glucose monitoring is important .
