IntroductionEndometriosis ( EM) is a frequently disease. Ten percent of women at bearing age suffer from the disease. The mechanism of EM is not yet expounded thoroughly. Although the theory of endometrium - planted has been accepted by most scholars, the standpoint of menstrual hemorrhage counter current cannot explain the phenomenon that few of them are inflicted with the disease, when ninety percent of women may exist menstrual hemorrhage counter current. As the development of immunology, the role of the immune factors in the occur and development of endometriosis is attached more and more importance. The immune abnormality of the patients with endometriosis has direct or indirect effect on plantation, adhesion and hyperplasia of ectopic endometrial tissue. Leptin can enhance cellular induced immune inflammatory reaction , then result in the immune abnormality ; as the angiogenesis, leptin can promote the formation of newborn blood vessels. The levels of leptin in peritoneal fluid ( PF) of the cases with endometriosis were detected by radioimmunoassay in this research. The objective of this research is to investigate the role of leptin in immunology and angiogenesis and inquire into the relationship between it and the etiopathology of endometriosis.Materials and methods1. Recourses of materialsTrial group: We examined 32 specimens of peritoneal fluid withEM, which came from the patients with EM treated at the department of gynecology in the Second Affiliated Clinical Hospital of the China Medical University between Jun. 2001 and Jan. 2002. We adopt the diagnosis criterion of EM in the national textbook of Gynecology and Obstetrics (5th edition) and employ the clinical phase measurement of EM provided by AFS. 14 cases of them are at early stage ( I ~ II phase) and 18 cases are at advanced stage ( M ~ IV phase). According to focus types, 10 cases have red focus and 22 cases have other types. All cases are graded according to the standard of dysmenorrhea grading as follows: Ono dysmenorrhea, 1 dysmenorrhea, no need for medicine and having no affect on daily living and general work, 2 dysmenorrhea, need for medicine and having affect on usual living and general work, 3dysmenorrhea, need for medicine, having affect on daily living and being unable to work. The ages of these casesrange from 26 years old to 47 years old ( AVG: 35. 3 5.4 years old). Body - mass index( BMI) is from 20.0 to 24. 8 ( AVG: 22.6 2.0).Control group: we also examined 5 specimens of peritoneal fluid, which came from the patients who received reversal of ligated tubes at the department of gynecology in the Second Affiliated Clinical Hospital of the China Medical University during the same period and 7 specimens that came from the patients who received tubal ligation at the service.center of family planning in Anshan . The cases totaled 12. The ages of these cases range from 32 years old to 43 years old ( AVG : 3 7 . 3 3 . 1 years old ) . Body - mass index is from21. 4 to 24.7 (AVG: 22.9 1.2).All patients have no metabolic diseases and acute inflammatory diseases. They all did not receive hormone therapy in the 3 months before operation. In addition, the menstrual period of every case was%quite regular and the operation of every case was carried during follicle phase. There are no significant difference between the age and BMI of trial group and control group (P >0.05).The collection and preservation of specimen: all specimens of PF in both trial group and control group are collected in operation. PF(2 ml) wa s sucked in recto - uterine pouch by aseptic syringe after peritoneum was cut open carefull laparoscopic operation, PF (2 ml) was sucked by aseptic suction tube after laparoscope was put in. In lapa-rotomy, we putted the specimen into centrifugal tubes which were cen-trifuged at the rate of 2000 r/min , then extracted the fluid and putted them into tube which were preserved in refrigerator under the temperature of -70C, If blood was mixed into some specimen, the specimen was abandoned.The tria
