Professor Wei Beihai is a state-renowned doctor of traditional Chinese medicine, who was among the first practitioners of Western medicine learning traditional Chinese medicine in the country. He is a clinical expert on spleen and stomach diseases, who has made significant achievements in curing internal miscellaneous diseases based on his theory of Weichang Fuyuan. The author of the following article will summarize Professor Wei’s academic thoughts and clinical experiences. This article has five parts.Part one is a general statement of this academic origin. This part focuses on the review of the formation of spleen and stomach theory, summarizing important understandings and views on spleen and stomach diseases by modern-time senior practitioners of traditional Chinese medicine, who showed insignificant difference in medicinal practice. The theory can mainly be described as comprehensive and general, which includes a lot of progress in modern medical researches. Through the researches of professor Wei Beihai, especially Spleen and Stomach Theory on the formation of his academic thoughts, and clinical experiences from professor Guan youbo, I summarize the sources of Professor Wei Beihai’s academic thoughts.Part two is a summary of Professor Wei Beihai’s medical theory. Professor Wei proposed a new diagnostic concept of the syndrome of spleen deficiency, hence coming up with basic concepts of Weichang Fuyuan Therapy. In the narrow sense, Professor Wei establishes treatment method of taking spleen qi upward and stomach qi downward as the general theory guiding the treatment of spleen and stomach diseases. In the wide sense, Weichang Fuyuan Therapy is used to guide the treatment of internal difficult and complicated diseases. Peitushengjinfa is just an important theory derived from the above mentioned concepts to treat COPD. As to spleen and stomach diseases, Professor Wei emphasizes Chronic Diseases Transforming to Collaterals, using activating blood circulation and removing stasis to enhance treatment. As to the medicines, Professor Wei believes in high doses and its strong effects, applies paired herbs, and is guided by the modern pharmacological researches. He pays special attention to food therapies as auxiliary treatment.Part three is a summary of Professor Wei’s clinical experiences. In this part, the author systematically summarizes Professor Wei’s deep understandings and clinical experiences in the treatment of chronic atrophic gastritis, peptic ulcer, gastrointestinal disorders, chronic colitis, chronic pancreatitis and other common diseases of the digestive system, as well as the chronic lung diseases, chronic fatigue syndrome, and exogenous febrile diseases. The proved cases were provided.Part four is the analysis of Professor Wei’s therapies on Jianpiheweifang, Jianpizhixiefang.Part five is the core of this article. By applying therapies of warming yang, nourishing yin, purgation, heat-clearing and detoxicating, I studied the guidance of Professor Wei’s Weichang Fuyuan Therapy to treat sepsis bowel dysfunction.Methods:Using method of retrospective study, Sixty patients in Emergency Department or ICU from October 2008 to June 2011 were enrolled and randomized into the treatment group and control group, with 30 patients in each group. All the patients were diagnosed sepsis, with symptoms of intestine dysfunction. According to traditional Chinese medical theory, these patients were deficiency of spleen-yang and kidney-yang, retention of turbid and toxicity. They were treated in accordance with Serious Sepsis and Sepsis Shock Guideline, at the same time using herbal medicine through nasal feeding, oral taking, or enema. APACHE II, bowel dysfunction, and changes of inflammatory reactions before and after treatment were observed. The data was analyzed by statistics.Results:Sixty patients were aged at 74.8±11.4. Initial diagnosis at emergency: thirty-seven Sepsis, three Acute cerebral vascular disease, three CRF, eleven AECOPD, four CHF, two patients after CPR. The clinical observations showed that after treatmant there is significantly different in improving intestinal functions, and clear significantly different from that in the control group and APACHE II and inflammatory response indicators after treatment were obviously dropped.Conclusions:Traditional Chinese medicine today believes that clearing heat and purgation is the appropriate therapy for sepsis bowel dysfunction, and their recovery is substantial. But Professor Wei take inspiration from Chengqi prescription of Yangming disease in Treatise on Febrile and Miscellaneous Disease,he believe that using clearing heat and purgation should take care of the stomach Qi. If taking attacts too much, it will be harmful for the function of stomach, "there is bit stomach Qi, and there is bit vigor", it is very important for the patients in the crisis. So clearing heat and purgation get involved in Weichang Fuyuan therapy,which means to expell the evil and reinforce the vital energy. Therefore this therapy includes the principle of purgation, heat-clearing and detoxicating remedy with warming yang and nourishing yin. The therapeutic effects of this therapy is definite and proved in the clinic.Achievements:Modern medicine researches prove that intestine is the core organ in MODS. But as mentioned before, Western medicine has limits in the treatment of this disease, with no thorough medical solution up to now. After a complete analysis of Professor Wei’s clinical thoughts and experience, the above mentioned therapy is established. Researches have proved that it has confirmative effects on the treatment of sepsis bowel dysfunction, which makes up the limits of modern medicine. Further evidence-based medical study is required to put the therapy into wide practice. Purpose:to discuss the guidance effects of Professor Wei’s Weichang Fuyuan Therapy to treat sepsis bowel dysfunction.Methods:Using method of retrospective study,Sixty patients in Emergency Department or ICU from October 2008 to June 2011 were enrolled and randomized into the treatment group and control group, with 30 patients in each group.All the patients were diagnosed as sepsis, with symptoms of intestine dysfunction. According to traditional Chinese medical theory, these patients were deficiency of spleen-yang and kidney-yang, retention of turbid and toxicity. They were treated in accordance with Serious Sepsis and Sepsis Shock Guideline, at the same time using herbal medicine through nasal feeding, oral taking (Radix Ginseng, Radix Aconiti Lateralis Preparata, Fructus Schisandrae Chinensis, Radix et Rhizoma Rhei Palmati, Radix Pulsatillae, Herba Patriniae Scabiosaefoliae), or enema (powder of Radix et Rhizoma Rhei Palmati, mirabilitum dehydratum). APACHE II, bowel dysfunction, and changes of inflammatory reactions before and after treatment were observed. The data was analyzed by statistics.Results:Sixty patients were aged at 74.8±11.4. Initial diagnosis at emergency: thirty-seven Sepsis, three Acute cerebral vascular disease, three CRF, eleven AECOPD, four CHF, two patients after CPR. This therapy is effective in clinic by pointing out the core pathogenesis of the disease. Traditional Chinese medicine today believes that clearing heat and purgation is the appropriate therapy for sepsis bowel dysfunction, and their recovery is substantial.But Professor Wei take inspiration from Chengqi prescription of Yangming disease in Treatise on Febrile and Miscellaneous Disease, he believe that using clearing heat and purgation should take care of the stomach Qi.
