Diagnostic Value of Colour Dopple Ultrasonography in Chronic Acalculous Cholecystitis

Objective To survey the diagnostic feasibility and significance of the Color Dopple Ultrasonography with high definition and high sensitivity in chronic acalculous cholecystitis.Methods To select 87cases with gallbladder wall thickening(≥3mm)examined by the 2-dimension ultrasound,among which 47 patients were chronic hepatitis.All the patients in out-patient and in hospital came from the affective department in The First Hospital affilliated to The ShanXi Medical University and The Coal Central Hospital.The age was 25-55.The 87 cases were divided into 3 groups:31 cases were in chronic acalculous cholecystitis(group CAC):there was no calculus in the gallbladder and bile duct,only the thickening gallbladder wall,and the gallbladder contractive functions letdown in the fat meal test;28 cases were in chronic calculous cholecystitis(group CCC):when examined by the 2-dimension ultrasound,there were calculus in the gallbladder and the gallbladder wall was thicker,but the size of the gallbladder was normal;28 cases were in the simple gallbladder wall thickening(group TCW),there was no calculus in the gallbladder and bile duct,only the thickening gallbladder wall,and the gallbladder contractive functions were normal in the fat meal test.30 cases were in the normal control(group CON).We utilized the LOGIQ-7 Color Doppler Ultrasonography with the 3~6Mhz frequency conversion probe.We measured the maximum flow velocity(Vmax)and the resistant index(RI)of cystic arterial blood flow in the gallbladder and hepatic arterial blood flow in all the patients.We made a statistic analysis in group CAC、CCC、TCW、CON by the 11.0 statistic software.Results 1.2D-ultrasound showed hyper-resonance of the hepatic parenchyma in the patients with hepatitis,and diffuse graticule alteration with the micro-tuberculation in parts of the patients.The resonance was normal in the patients of non-hepatitis.the There were no significant disparity in average gallbladder wall thickeness and the size of the gallbladder among group CAC、group CCC、group TCW and group CON. 2.The Vmax and RI of cystic artery in the group CAC and group CCC were evidently higher than those of the normal,there were apparent differences(t=11.943,0.114,P<0.05; 11.157,0.028,P<0.05).But between the two groups the Vmax and RI had no difference (t=1.085,0.019,P>0.05).The Vmax and RI of cystic artery had no difference between group TCW and normal(t=0.786,0.100,P>0.05).The Vmax and RI of cystic artery in the cholecystitis caused by chronic hepatitis had no difference with those of the cholecystitis without hepatitis(t=4.226,0.061,P>0.05).3.The Vmax and RI of hepatic artery had no difference among group CAC、group CCC and group CON,but the Vmax and RI of the three groups were evidently higher than those of the normal,there were apparent differences(P<0.05).But the Vmax and RI of hepatic artery of the cholecystitis caused by chronic hepatitis were evidently higher than those of the cholecystitis without hepatitis,there were apparent differences(t=14.700,0.108,P<0.05; 10.471,0.100,P<0.05).The Vmax and RI of hepatic artery had no difference between group cholecystitis without hepatitis and normal(t=4.226,0.003,P>0.05).Conclusions 2D-ultrasound showed the thickening-wall of gallbladder,the measurement of cystic arterial flow with CDFI has an important value in the diagnosis of chronic acalculous cholecystitis.The Vmax and RI of cystic artery were higher in the chronic acalculous cholecystitis.Meanwhile detecting the hepatic artery and measuring the Vmax and RI,it was helpful of the differential diagnosis between the cholecystitis caused by chronic hepatitis and those of the cholecystitis without hepatitis
