Objective To explore the rat heterotopic small bowel transplantation and perioperative management experience , rat spleen and intestinal combined transplantation of animal models . Method microsurgical technique , the entire cut for intestinal small bowel , portal vein and the abdominal aorta with superior mesenteric artery proximal . Spleen and intestine joint transplant recipients , including the spleen and small intestine , abdominal aortic reserves the superior mesenteric artery , splenic artery branch of the celiac artery , portal vein splenic vein branch reserved . Spleen intestinal surgery situ perfusion and intestine lavage . Cycle reconstruction : arterial anastomosis donor with the superior mesenteric artery and celiac artery branch of abdominal aortic segment spleen and the recipient abdominal aortic end side anastomosis ; venous anastomosis using micro- tube technology will be available for intestinal portal vein and the recipient left renal vein anastomosis . The intestinal graft abdominal wall at both ends of double stoma . Surgery for receptor tail skin incision , trocar tail vein puncture infusion, to rewarming , antibacterial treatment after surgery . Results 165 surgeries , 38 of them familiar with the anatomy and surgical skills training . Pre-experiment 86 times transplantation, in which the spleen and intestinal combined transplantation 20 . The formal experiment 41 times transplantation, spleen and intestinal combined transplantation 8 , 30 survived more than 10 days , and completed the experimental study , the success rate of 73.2% . Reason for the failure : arterial anastomotic bleeding 2 and two abdominal infection ascites , the arterial anastomotic thrombosis transplanted intestinal necrosis 2 , 3-5 days after transplantation intestinal bleeding 5 . Cold ischemia time control within 1 hour of warm ischemia time of 24 ± 11 minutes, and the entire procedure for one person to operate , the operation time of about 4 hours . The conclusions for intestinal vascular anastomosis technique , intraoperative blood volume to maintain , as far as possible Less lt ; WP = 9 gt ; less warm ischemia time is the key to the success of small bowel transplantation in rats . With celiac artery splenic artery and the superior mesenteric artery segment of the abdominal aorta and donor abdominal aorta end to side anastomosis ; consistent with splenic vein , portal vein and the donor left renal vein casing surgical methods , to establish a stable rat spleen and intestinal combined transplantation of animal models .
