Objective:Research to the mechanism of the antagonism on brain injury and encephaledema in acute intercerebral hemorrhage by acupuncture.This topic revealed the correlated mechanism of the antagonism on brain injury and encephaledema in the rat of acute intercerebral hemorrhage through observing the influence of the histomorphology and the expression of GDNF,MMP-9,AQP-4 and VEGF in brain tissue by BaiHui-QuBin scalp-acupuncture,and paved the experimental foundation for the clinical treatment of acute intercerebral hemorrhage.Methods:Selected the healthy Wistar male rats of 320(among the total 160 rats were used detecting the brain histomophology,immunohistochemistry and in situ hybridization;another 160 rats were used detecting the brain water content only)by examination.The rats were assigned into 3 sets randomly:the model set,the pinprick set,the western medicine set,and contained 100 rats in each.Each set were again divided into five subsets of 6 hours,1 day,2 days,3 days,7 days,and contained 20 rats in each.All rats were made to be the intercerebral hemorrhage model.Another 20 rats were assigned as the matched blank set.The rats of the pinprick set were acupunctured on the trouble side by BaiHui-QuBin scalp-acupuncture.We observated the changes of the brain histomophology of each set with light microscope and electron microscope;detected the brain water content of each set with the method of dry-wet weight;detected the positive expression of GDNF,MMP-9,AQP-4 and VEGF in brain tissue of each set with the methods of immunohistochemistry and in situ hybridization.Results:1.It was discovered that the model set’s brain tissue surrounding the hematoma was necrosis,dropsy,karyopyknosis and vaculization of never cells,infiltration of inflammatory cells and dilating and congestion of blood capillary with the method of observating under light microscope.The subset of 3 days was most serious.Compared with the model set,the brain tissue of the pinprick set repaired gradually,edema and infiltration of inflammatory cells lightened.Compared with the model set,the changes of the western medicine set was relatively showed no significant.2.It was discovered that the model set’s brain tissue surrounding the hematoma was swelling of the nerve cells,agglutination of chromatin,solution of nucelus, vacuolization of bioblast,split of cristae,dilatation of rough endoplasmic reticulum,depolymerization of polysome and engorgement of synaptic bouton with the method of observating under electron microscope. Compared with the model set,the damage of the pinprick set was abatement. Compared with the model set,the changes of the western medicine set was relatively showed no significant difference.3.All of the rats after being made model appearanced the phenomenon of encephaledema of different degree.The rats of the model set appearanced the phenomenon of encephaledema lightly in 6 hours and to the peak in 3 days and still to remain lightly in 7 days.The phenomenon of encephaledema of the rats of the pinprick set was abatement compared with the model set and the western medicine set.From 1 day to 7 days,compared with the model set,the difference was manifest(P<0.01).From 1 day to 3 days,compared with the western medicine set,the difference was manifest(P<0.01).The phenomenon of encephaledema of the rats of the western medicine set was abatement compared with the model set from 1 day to 3 days(P<0.01).4.The detected result of GDNF with the method of in situ hybridization:All of the rats after being made model appearanced the phenomenon of multiplication of expression of GDNFmRNA.The rats of the model set appearanced the phenomenon of multiplication of expression of GDNFmRNA visiblely in 6 hours and to the peak in 1 day and descending after 2 days and to near the normal in 7 days.In the same time phase point,Compared with the model set and the western medicine set,the pinprick set’s expression of GDNF was higher obviously and relatively showed the significant difference and had the statistical significant(P<0.01),and still to remain higher than blank set in 7days.Compared with the model set,the difference of the western medicine set had no statistical significant(P>0.05).5.The detected result of MMP-9 with the method of immunohistochemistry:All of the rats after being made model appearanced the phenomenon of multiplication of expression of MMP-9.The rats of the model set appearanced the phenomenon of multiplication of expression of MMP-9 visiblely in 6 hours and to the peak in 2 days and descending gradually after 3 days and still to remain higher than blank set in 7 days.The pinprick set’s expression of MMP-9 was lower compared with the model set and the western medicine set.In the same time phase point,compared with the model set,the pinprick set’s expression of MMP-9 was lower obviously and relatively showed the significant difference and had the statistical significant(P<0.01);from 6 hours to 2 days,compared with the western medicine set,the difference was manifest(P<0.01).The phenomenon of multiplication of expression of MMP-9 of the rats of the western medicine set was lower compared with the model set from 1 day to 7 days(P<0.01).6.The detected result of AQP-4 with the method of immunohistochemistry:All of the rats after being made model appearanced the phenomenon of multiplication of expression of AQP-4.The rats of the model set appearanced the phenomenon of multiplication of expression of AQP-4 in 6 hours and to the peak in 3 days and still to remain higher than blank set in 7days.The pinprick set’s expression of AQP-4 was lower compared with the model,set and the western medicine set.From 1 day to 3 days,compared with the model set and the western medicine set,the pinprick set’s expression of AQP-4 was lower obviously and was relatively showed the significant difference (P<0.01).Compared with the model set,the western medicine set’s expression of AQP-4 was relatively showed no significant difference except 2 days’s subset(P<0.05).7.The detected result of VEGF with the method of immunohistochemistry:All of the rats after being made model appearanced the phenomenon of multiplication of expression of VEGF.The rats of the model set appearanced the phenomenon of considerable multiplication of expression of VEGF in 6 hours and to the first peak,from 1 day to 2 days appearanced the descending tendency,and to the peak in 3 days suddenly and still to remain higher than blank set in 7 days.From 1 day to 3days, compared with the model set,the pinprick set’s expression of VEGF was lower obviously and were relatively showed the significant difference and had the statistical significant(P<0.01);however,the pinprick set’s expression of VEGF was higher obviously in 7 days compared with the model set(P<0.01).Compared with the model set,the western medicine set’s expression of VEGF was higher obviously and was relatively showed the significant difference in 7 days(P<0.01).Conclusions:1.The therapy of BaiHui-QuBin Scalp-acupuncture can relieve the injury of nerve cell and the destruction of ultrastructure obviously,and decrease the infiltration of inflammatory cells,and cut down the extent of encephaledema around the hematoma in acute intercerebral hemorrhage,so prompt having the function of antagonism on brain injury and encephaledema.2.This method possibly through the way of promoting the expression of endogenous GDNF and inhibiting the expression of endogenous MMP-9 and endogenous AQP-4,and inhibiting the expression of endogenous VEGF in the earlier and promoting the expression of endogenous VEGF in the advanced stage,and cutting down the brain water content and so on have the function of antagonism on brain injury and encephaledema.This method possibly have the function of two-ways regulation in acute intercerebral hemorrhage.It’s therapeutic effect surpass the Aniracetam.
