A Study on the Rate of Potential Rabies Exposure and Determinant Factors of Rabies Vaccination Among the Rural People Who Came from Four Counties in Hunan Province

Objective:To understand the rate and characteristics of potential rabies exposure to the rural residents in Hunan province, and to explore the determinant factors of post-exposure rabies vaccination among the crowd, thus we could find the high risk population of rabies exposure and the key determinant factors of post-exposure rabies vaccination, in the hope to provide scientific evidence for strategy making to prevent and control the rabies.Methods:By taking the multistage sampling method, questionnaire survey was conducted in forty villages which belonged to four counties’twenty towns in Hunan province. Samples of these villagers were selected from each village for their situation of the knowledge of rabies prevention and cure, the family basic information, and post-exposure disposal information. Then calculate the rate of rabies exposure, post-exposure wound disposal, rabies vaccination and passive immunity preparation injection among the investigators during January1st,2009to September30st,2010. Univariate analysis and multivariate non-conditional logistic regression analysis were used targeting at the factors of post-exposure rabies vaccination.Results:(1) Among3042villagers being interviewed,124person-times exposure were found during January1st,2009to September30st,2010, The total exposure rate in the observed villagers was4.08%, the years average exposure rate was2.33%. Whether classified by gender or region, the trend of accumulative exposure rate in each age group was both similar:the rate was highest from0to9age group and lowest in20to29age group, then gradually ascend, which presented a "V" shape distribution. Quadratic equation (Y=bo+b1X+b2X2) can be used in the proper description of age-specific exposure rate trend. The trend among the female exposures was fitted well by quadratic equation (R2=0.895).(2)Among124person-times rabies exposures,35person-times (28.2%) treated their wounds by themselves after exposure,93person-times (75.0%)received treatment from medical institutions or clinics,89person-times(71.8%) were immunized and6person-times(8.3%) among72person-times grade III exposures received antiserum or immunoglobulin of rabies.(3)Univariate analysis showed that the post-exposure rabies vaccination was statistically link to such six factors as exposure sites, the score of rabies prevention knowledge, whether or not to take part in the new type of rural cooperative medical care, the disposable cash income in the year of2009, the distance from family to health clinics in towns and townships, the county which the exposed person belonged to. Logistic regression analysis demonstrated that four factors including exposure sites, the score of rabies prevention knowledge, the disposable cash income in the year of2009, the county which the exposed person belonged to had a remarkable determinant on the rate of post-exposure rabies vaccination.Conclusion:(1) The rate of potential rabies exposure in four counties of Hunan province is very high, children and the elderly are the high risk population of rabies exposure.(2)The rate of standardization treatment of wounds after rabies exposure is very low, the rate of rabies vaccination and passive immunity preparation injection are both have a big distance from the demand of "The work norm of treatment of rabies post-exposure in2009".(3)Exposure sites, the score of rabies prevention knowledge, the disposable cash income in the year of2009and the county which the exposed person belonged to are four key affected factors of post-exposure rabies vaccination.
