Clinical Observation on the Effect of Dahuang-tongfu Therapy on Lung Fluid and Inflammation of Severe Pulmonary Contusion Patients

ObjectiveTo investigate the clinical effect of Dahuang-tongfu therapy on lung fluid and inflammation in severe pulmonary contusion patients.MethodsCollect60cases of severe pulmonary contusion patients according to the inclusion and exclusion criteria. This prospective clinical research is a randomized control trial. Samples have been randomly attributed to two groups, in which, one is Traditional Chinese Medicine group (30cases) receiving conventional treatment and Dahuang capsule and the other is control group (30cases) receiving conventional treatment and placebo. All Subjects have been monitored at the1st,3rd and5th days in ICU, by surveying inflammation factors, comprised of TNF-α、IL-6and IL-8; and lung fluid metabolism index of EVLW, with WBC, Lac and APTT as well. Pulmonary contusion scores have also been compared between groups before and after treatment.ResultsAccording to dropout and elimination criteria, the final available cases are56, including28cases in TCM group and28in control group. The before and after treatment differences in pulmonary contusion scores, APTT and IL-8between two groups have no statistics significance(P>0.05). The before and after treatment differences in WBC, Lac, TNF-α, IL-6and EVLW between two groups are statistically significant(P<0.05).ConclusionThe Dahuang-tongfu therapy can restrain progress of inflammation response by reducing TNF-α and IL-6of severe pulmonary contusion patients, of which, the therapy also can reduce both lung fluid and pulmonary edema. The TCM group for IL-8was not significantly reduced, thus the effect on IL-8is not clear.
