Hip Arthroplasty for Treatment of Ankylosing Spondylitis in18Cases

Objective:To analysis the results of follow-up and clinica data in patients with ankylosing spondylitis(AS) after total hip arthroplasty(THA) and discuss preoperative and postoperative management of THA. the present study is to improve the treatment skill for AS through sharing our experiences.Patients:From2001to2011,18AS patients(28hips) with tetanic deformity were performed with primary total hip arthroplasty, There are16male cases with25hips and2female cases with3hips, and their ages ranging from18to54years and an average age of31years old. They are all have an average follow-up period of34months (range,8-56months).They all fulfilled the dianostic criteria of ankylosing spondylitis(New York criteria of AS,1984). The Harris hip scores before THA operation were12to56and the average scores were38.9.Methods:total hip arthroplasty was performed with a lateral approach. Noncemented prosthesis used in all patients. Antibiotics and anti DVT was given postoperative and the patients were advised to do systemic healing exercise. A follow-up for8to56(mean34)months was followed. The Harris score was carried out in all patients to evaluate the hip function.The clinical outcome was evaluated with radiologic examination.Results:According the follow-up data, incision infection, DVT, prosthesis loosening and injury of nerve and vessels have not be found in all patients. There was no dislocation and no revision was performed. Pain was totally relieved in22patients(78.9%). Preoperative average total range of movement of hip was13.6°. The postoperative was146.6°. Based On Harris hip score, preoperative average scores were39.9and postoperative scores were80.6. Heterotopic ossification was found In2 cases(3hips) and Brooker grade IConclusion:Total Hip Arthroplasty(THA) is an effective treatment to the AS patients with titanic deformity. THA can significantly rectify the rigidity, relieve the pain, restore the function and improve the postoperative patients life quality.
