As a common disease, Lumbar Intervertebral Disc Herniation (LIDH) has brought heavy burden to family and the society. As one kind of non-surgical Conservative therapy to LIDH, Jiaji Electroacupuncture (EA) is popular and it is widely accepted. As the research on LIDH go, Jiaji EA has accepted more and more attention. The research on its possible mechanism has obtained certain result. As the important pathological basis of LIDH, the pathological mechanism of Lumbar Intervertebral Disc Degeneration(LIDD) has not been clarified completely. After The observation on the clinical therapeutic effects of Jiaji EA to LIDH, this subject trys to research the possible mechanism of the Jiaji EA’s regulation on lumbar disc degeneration.Part1The Observation on the Clinical Therapeutic Effection of Jiaji EA to LIDHObjeetivesIn order to Observation the Clinical Therapeutic Effection of Jiaji EA to LIDH, the research choose three different methods to evaluat the Clinical Therapeutic Effection of Jiaji EA to LIDH after referring to a large number of Clinical research.MethodsAccording to the therapy,60Patients with LIDH are randomly divided into two groups:group with Jiaji EA (treatment group n=32n) and group with ordinary EA(control group n=28), for two weeks’ treetment. After observing signs and symptoms of the patients in two group before and after treatment, to evaluat the Clinical Therapeutic Effection using three different methods1. Traditional comprehensive method to evaluate the effectionTo evaluate the effection accoding to the traditional comprehensive method.2.Pain-quantified method to evaluate the effectionTo evaluate the improvement of pain before and after treatment using the internationnlly recognized McGill pain questionnaire(MPQ), which measures pain from different aspects.3. Scoring method created by Yangshi to assess the effectionTo assess the therapeutic effect using the scoring method created by Yangshi. Accoding to the following four parts closely related to LDH, such as symptoms and main complaints, the abilities of work and life, the function of lower limb, clinical symptom, then, add the score, the little of the score, the worse of the therapeutic effect.Results1. Using the traditional comprehensive method to assess the efficacy, compared with control group, the markedly effective rate of the treatment group and the total effective rate are both higher than those of the control group (P<0.05, P<0.05). 2.Using the pain-quantified methodto assess the efficacy, total points of the treatment group is lower than that of the control group (P<0.05); the total score of pain indicators of the patients in the two groups both reduced after the treatment. The markedly effective rate of the treatment group and the total efficiency is significantly better than the control group (P<0.05, P<0.05).3. Using the Scoring method created by Yangshi to assess the effection, the total score of the treatment group is higher than that of the control group after the treatment (P<0.05); the total score of the patients in the two groups both reduced after the treatment (P<0.05, P<0.05). Excellent rate of the treatment group is higher than that of the congtrol group(P<0.05); The excellent rates of the two groups both rise after the treatment (P<0.05, P <0.05).Conclusions1. Both of the two methods are effective to LDH.2. The comprehensive efficacy and effect to control pain of the Jiaji EA group are better than those of the ordinary EA group. As methods to treat LDH, Jiaji EA is better to improve the clinical symptoms and signs.Part2The Effection of Jiaj i Electroacupuncture on the Expression of Bcl-2/Bax Protein in the Degenerative Lumbar Intervertebral Disc Tissue of the Model RabbitObjeetivesIn order to investigate the the mechanism that Jiaji EA’s regulation on LIDD, observe the expression of Bcl-2and Bax protein in the degenerative lumbar intervertebral disc tissue of the model rabbit and the effection of Jiaji EA on the Expression of Bcl-2/Bax protein in the degenerative lumbarMethedsAfter selecting4rabbits to the normal group from the28New Zealand white rabbits randomly,The remaining24rabbits were randomly divided into three groups, they were false model group, model group and EA group,8rabbits each. The rabbits in the normal group accept accept MRI test at the1th day directly, then take the intervertebral disc tissue of the corresponding Lumber for test. The rabbits in model group accept production of LIDD model without any treatment. The rabbits in the false model group almost accept the same processing but without the Intervertebral pressure,also without any treatment. The rabbits in the EA group accept the EA treatment at the first day after the successful modeling. The modeling last28days, the treatment last28days. The rabbits of the three groups accept MRI test of the corresponding lumber intervertebral disc at the28th day and56th day. After the MRI test, we selected4rabbits randomly from each group and take the intervertebral disc tissue of the corresponding Lumber at the28th day and56th day. All of the lumber intervertebral disc tissue were sent for Western Blot test about Bcl-2/Bax protein.Results1. The surgery of model ing rabbits went well with little blood loss and surgical injury. The rabbits were ok with well heel ing incision, with no obvious paralysis and significant toilet incontinence. The survival rate is survival ideal.2. The MRI test of the rabbits in model group and EA group show deneneration sign.3. At the28th day, Compared with the false model group,the Bcl-2/GAPDH, Bax/GAPDH of The Intervertebral disc tissue of the rabbits in model group and EA group show statistically significant difference (P<0.05). Combined with the result of MRI examination which pointed significant degeneration, it showed that Bcl-2protein expression decreased and Bax protein expression rised in the degenerative lumbar Intervertebral disc tissue.4. At the56th day, the Bcl-2/GAPDH, Bax/GAPDH of The Intervertebral disc tissue of the rabbits in the model group and EA group show statistically significant difference (P<0.05). The Bcl-2protein expression rised and the Bax protein expression decreased in the degenerative lumbar Intervertebral disc tissue of the rabbits in EA group.It showed that Jiaji EA could raise the levels of bcl-2protein and decrese the levels of bax protein in the degenerative lumbar disc tissue of model rabbits.Conelutions1.Execute The transform may increase the sensitivity of the cell to apoptosis and decrease the ability to antagonize apoptotic.then, the rate of the apoptotic increased. All of these transform resulted in LIDD. intervertebral pressure to the New Zealand white rabbits was a ideal animal models of LIDD with ideal success rate、 operability and operability.2. The Bcl-2、Bax protein involved in the process of LIDD. The bcl-2protein Increased and the Bax protein raised in the degenerative lumbar disc tissue of rabbit model. The transform may increase the sensitivity of the cell to apoptosis and decrease the ability to antagonize apoptotic. then, the rate of the apoptotic increased. All of these transform resulted in LIDD.3. Jiaji EA could raise the levels of bcl-2protein and decrese the levels of bax protein in the degenerative lumbar disc tissue of model rabbits. The transform may decrease the sensitivity of the cell to apoptosis and increase the ability to antagonize apoptotic, which may be the channel that Jiaji EA regulate LIDD.
