Objective:To discuss the influence on each index of blood gas analysis and the kidney in patients with acuteexacerbation of chronic obstructive pulmonary disease(AECOPD) through early treatment of non-invasivepositive pressure ventilation (NIPPV). In the end of to explore the best choose of density of oxygen.Method:80patients with AECOPD were accepted to non-invasive positive pressure ventilation treatment andregular medical treatment. They were randomly divided in to four groups: group A(FiO225%)、groupB(FiO2is35%)、group C(FiO245%)、 group D(FiO255%).Each group of20case.Compare of thechanges of blood gas analysis、PaO2、PH、and PaCO2were observed in the four groups before and afterin use of non-invasive ventilation in2hours、1、3、5、7days in the same time.Using ELISA method to detectrenal injury molecular-1(KIM-1) levels, while monitoring acute physiology and chronic health Ⅱ(APACHE Ⅱ) score, serum creatinine (SCr), β2-microglobulin (β2-MG), serum these inhibitory elementC (CysC), trace albumin (mAlb), urea nitrogen (BUN), uric acid change.Result:1. With the condition of non-invasive ventilation and suction different the density of oxygen, Thecomparison of the result of blood-gas analysis with four groups had no statistical significance (P>0.05).The changes of PH、PaO2、PaCO2had obviously decline than before mechanical ventilation, and showedstatistical significance (P<0.05).2. With the condition of non-invasive ventilation and suction different the density of oxygen, the changes ofrenal injury molecular-1(KIM-1) with four groups had statistical significance (P<0.05). Compared withrenal injury molecular-1(KIM-1),The groups of B、C had obviously downward trend than the group of A、D.Compared with different time points of the same group, renal injury molecular-1(KIM-1)had obviouslydecline than before mechanical ventilation, and showed statistical significance (P<0.05).3. With the condition of non-invasive ventilation and suction different the density of oxygen, the changesof serum creatinine (SCr), trace albumin (mAlb), urea nitrogen (BUN), uric acid、serum or element C(CysC)、 β2-microglobulin(β2-MG)with these four groups had no statistical significance (P>0.05).Compared with different time points of the same group, serum creatinine (SCr), serum or element C(CysC)、 β2-microglobulin(β2-MG)、trace albumin (mAlb), urea nitrogen (BUN), uric acid had obviouslydecline than before mechanical ventilation, and showed statistical significance (P <0.05).Conclusion:1.With the condition of non-invasive ventilation and appropriately controlled the density of oxygen, Thegroup of35%and45%the density of oxygen are more improve to renal function of patients.2. With the inhalation of oxygen time prolonged, hypoxia and renal function of AECOPD are improved, thecondition gradually improved.
