Effect of Emodin on Rat's Myocardial Damage Following Severe Acute Pancreatitis and Mechanism

ObjectiveTo investigate the effect of Emodin on severe acute pancreatitis (SAP)associated myocardial damage in rats, so as to better understand the role and itspossible mechanism.MethodsThirty-two health adult male Sprague Dawley rats were divided randomly intosham-operated group (group A); SAP group (group B); Emodin treatment group(group C);Emodin and5-HD(the mitochondrial ATP-sensitive potassium channelblocker)treatment group (group D). SAP model was induced by retrograde infusion of5％sodium cholate into the bile-pancreatic duct. After24hours later, artery blood ofgroup of each was obtained at induction of the model. The samples were taken andfixed，Enzyme-linked immunosorbent assay (ELISA) was used to determine proteincontent of cardial troponin I (CTnI)、 creatine kinase-MB (CK-MB)、 necrosisfactor-α(TNF-α), the automatic biochemical analyzer was used to detecte the expressionof amylase (AMY);SP immunohisrochemical technique was used to detcte theexpression of Bcl-2,bax in myocardial tissue,TUNEL technique was used to detectedmyocardial apoptosis. Pancreatic and myocardial tissue were stained with HE andexamined under light microscopy, the morphology of myocardial ultrastructure wasexamined under electron microscopy. ResultThe levels of serum CTnI、CK-MB in the Emodin treatment group were lower thanin the SAP group, but remain higher than in the sham-operated group(p<0.05), theEmodin and5-HD treatment group were lower than in SAP group, higher than in theEmodin treatment group(p<0.05). The levels of serum TNF-α in the Emodin treatmentgroup were lower than in the SAP group, there is no significant difference between theSAP group and the Emodin and5-HD treatment group (p>0.05).The levels of serumAMY in SAP group was the significantly highest than any other groups, the Emodinand5-HD treatment group、the Emodin treatment group、the sham-operated group inturn decrease(p<0.05). Bax in myocardial cells in group of the Emodin treatment werelower than in the group of SAP, were higher than in the sham-operated (p<0.05). Bcl-2in myocardial cells in group of the sham-operated is the highest expression, theEmodin treatment group, the Emodin joint5-HD treatment group and the SAP groupin turn decrease(p<0.05). The technique of TUNEL was used to detect myocardialapoptosis, the serious of apoptosis was remarked with apoptotic index (AI). The AI inSAP group was the significantly highest than any other groups, the Emodin and5-HDtreatment group、the Emodin treatment group、the sham-operated group in turndecrease(p<0.05). The damage of pancreatic and myocardial tissue in group theEmodin treatment group were lower than in the SAP group，but there is no differencesbetween the SAP group and the Emodin and5-HD treatment group.ConclusionEmodin could lighten the pathological changes of SAP rats, and alleviatemyocardial damage following SAP. It’s maybe through inhibit inflammatory mediatorsand Activate the mitochondria sensitivity potassium channel （mitoKATPC）againstcell apoptosis to achieve.
