Combination of Hematological Indicators for Influencing the Prognosis of Cerebral Infarction

Objectives Acute cerebral infarction (ACI) is a disease with high death anddisability rates. Epidemiological data in recent years showed that, cerebrovasculardisease in our country’s population is dangerous: the second cause of death, next only tomalignant tumors. An important principle of treatment of ACI is how to identifyhigh-risk patients earlier, then take a positive intervention measures to make patients getbetter clinical prognosis. Large differences between the prognosis of patients, so theprognostic judgment of cerebral infarction patients appears crucial. Characteristic serummarker has attracted much attention in recent years. With the knowledge updating, wehope to find a set of signature of test indicators in clinical trials with both sensitive andspecific to monitor treatment effect and prognosis, that can convenient clinical benefitof patients. The possibility of using a combination of biomarkers in this instituteexplored will or could be favourable to provides a new clue and thinking for the earlydiagnosis of ACI,monitoring therapeutic effect and prognosis evaluation assessment.Methods Experimental subjects come from the department of neurology in thefirst affiliated hospital of Da Lian Medical University between January2011andDecember2011, during hospitalization and follow-up of patients with cerebralinfarction. A total of96cases, among them,55cases male, female41cases, male tofemale ratio was1.34:1, aged40to92years old, average69.64±11.8years old. Allcases are in line with the guideline and diagnostic criteria of diagnosis and treatment ofacute ischemic stroke in China in2010. Selected control group is100cases of healthycheck-up with age and gender matched normal, among which56were male, female44cases, male to female ratio was1.28:1, aged40to86years, mean age65.38±12.46years old. Group them based on the modified Rankin scale to evaluate clinicalimprovement, corresponding to the discharge of prognostic criteria-in decrease of NIHSS percentage (%).Routine blood, blood fat, blood sugar, hepatic function, renalfunction, blood clotting, such as blood indexes were determined separately for each andstatistical analysis retrospectively.Using SPSS11.5software for processing. Twodifferences between group and group comparison use single factor analysis of variance(One-way ANOVA). Significant for analysis of single factor variables do multipleLogistic regression analysis to obtain serum marker function equation.Results①WBC、Fbg、APTT、Glu、K+、LDL、Cre、Hcy、h-CRP in cerebralinfarction patients were higher than those in normal level，but PLT、HDL、TP、Ca2+werelower than the healthy controls. Difference was statistically significant(P<0.05～0.01).②Through the correlation analysis,Ca2+、WBC、Glu、Fbg、LDL/HDL、h-CRPandK+become the main factors who affect prognosis of cerebral infarction.③Using of theseindicators,we get serum marker function equation corresponding the patients NIHSSpercentage (%)with cerebral infarction by stepwise regression analysis.Conclusion This study basing on analysis of the33laboratory testing serumindexes which may have an impact on the prognosis of patients with acute cerebralinfarction,discovered Ca2+、WBC、Glu、Fbg、LDL/HDL、h-CRP and K+within72hof admission hospital are all the influence factors of prognosis of acute cerebralinfarction. The level trend of these indicators is closely related to the occurrence,development and recovery of ACI. This study results suggest that, consisting of6kindsof serum index marker group can improve the prognosis of cerebral infarction on theaccuracy of the judgment. To achieve early discovery, early intervention, and dynamicmonitoring by the bed, this combinations of hematological indicators can provides ahigh sensitivity and specificity, simple and economic method,with a certain clinicalsignificance.
