Objective:The patients in Leukoaraiosis with lacunar infarction is common discovered during the man whose age is older than sixty-years in clinical, previous study considered that was caused by the chronic ischemic and low perfusion.This study is to investigate the incidence rate of extracranial or intracranial artery stenosis in patients whom in lacunar infarction group with Leukoaraiosis;the correlation of the severity of leukoaraisis with large vessel stenosis;and the risk factors of leukoaraisis, further to explore the possible pathogenesis for LA.Methods:Consecutive patients elder than40years were consecutively included in the Department of Neurology, Beijing Military General Hospital from January2012to October2012, all of them had accepted a Magnetic Resonance Imaging(MRI) and a head/neck CTA/MRA, They were divided into lacunar infarction group (infarction of diameter is3~20mm) with Leukoaraiosis (n=100) and lacunar infarction with non-Leukoaraiosis(n=100) according to their cerebral MRI findings. Vascular stenosis degree can be determined by the formula stenosis=[1-(D stenosis/Dnormal)]x100%.To compare the incidence rate of extracranial or intracranial artery stenosis in the two groups, analyze the correlation of the severity of leukoaraisis with large vessel stenosis. Local factors were records for the analysis.including age,sexual,smoking,drinking,High blood pressure, diabetes, Heart disease, Cerebral infarction/TIA, hyperlipidemia.Carotid artery stenosis/plaques, ACA/MCA/PCA stenosis.The SPSS16.0software were used for statistical calculations, and p<0.05.Results(1) The incidence rate of age, Heart disease.Carotid artery stenosis/plaques,and the level of SBP/DBP in patients with LI and LA are higher than LI which with non-LA;(2) There are29patients with carotid artery stenosis,5patients with ACA stenosis,6patients with MCA stenosis,2patients with PCA,1patients with basilar artery,the incidence rate of extracranial or intracranial stenosis is43percent in LI and LA.The other group has17patients with carotid artery stenosis,3patients with ACA stenosis,7patients with MCA stenosis,1patients with PCA.the incidence rate of stenosis is28percent. the two groups had significant difference,The incidence rate of extracranial or intracranial artery stenosis in patients with LI and LA is more frequently than LI (X2=4.038,P<0.05).(3) The severity of leukoaraisis with large vessel stenosis has positive correlation which is discovered by Spearman rank correlation in LI with LA (rs=0.83, r>0, P <0.001).(4) The diameter of the infarction is6-15mm in LI and LA,but it is0-10mm in LI,The comparetion of the two group has significant difference (t=-16.526,,P<0.001),which means that the diameter of the infarction in LI with LA is greater than the LI.(5) Binary Logistic regression shows age, history of hypertension, Carotid artery plaques/stenosis are included in the mode(OR=3.56,95%CI:1.40~9.07;OR=1.56,95%CI:1.04~2.34;OR=2.13,95%CI:1.05~4.31)Conclusion(1) The incidence rate of extracranial or intracranial artery stenosis in patients with LI and LA is more frequently than LI.(2) The severity of leukoaraisis with large vessel stenosis has positive correlation.(3) The diameter of the infarction in LI with LA is greater than the LI.(4) Age.High blood pressure and atherosclerosis/stenosis are the independent risk factors for LA(5) The extracranial or intracranial artery stenosis have participate the form of the LI and
