Investigation of the Effect of Fentynal after Subarachnoid Hemorrhage

Objective:To study Fentanyl sulfate on cerebral vasospasm after-subarachnoid hemorrhage and its mechanism of action.Materials and Methods:The subarachnoid hemorrhage animal model were made by tied up one side of the common carotid artery+and twice blood injection into the cisterna magna.72healthy adult rabbits were randomly divided into four groups A, B, C, D, The control group (A group), SAH group (B group), SAH animals treated with fentanyl intravenous (C group) and SAH animals treated with fentanyl intramuscularly group (D group).Then the four groups were divided into3subgroups according to the different time points,each subgroup contains6individuals. The animals of A group corresponding only to sham surgical procedures, then raised under standard rearing for24h,72h,7d,without subarachnoid injection of blood; The animals of B group were processed with intrathecal injection of blood twice, then raised under standard rearing for24h,72h,7d; The animals of C group were raised under standard rearing for24h,72h,7d after the SAH models were made, respectively, during those days Fentanyl was given by intravenous administration, twice a day,the second was one o’clock later after the first; The animals of D group were raised under standard rearing for24h,72h,7d after the SAH models were made, respectively, during those days Fentanyl was given by intramuscular administration (0.05mg/kg), twice a day,the second was one o’clock later after the first; at the time points oaccording to experimental design, A group, B group, C group and D group were gone neurobiology score, and at the time point according to experimental design brains were removed, at the two points after the optic chiasm for1.4cm and2.0cm the basar artery were obtained in coronal Plane and undergone HE staining to observe the basilar artery diameter; while taking hippocampus processed with HE staining and Bcl-2, Bax immunohistochemical staining to observe Bax and Bcl-2expression, SPSS13.0software was used to analyze experimental data.Results:The neurological function score after SAH in rabbits observed:A group:before the test,food and water intake was normal, without neurological deficit, only after two operations had a short-term decline in food intake, no neurological deficit; B, C, D groups after surgery appeared to reduce in food intake, and different degrees of neurological dysfunction especially after the blood injection for the second time, symptoms of B group was significantly heavier than C, D groups. HE staining observation of the basilar artery, In A group:the walls of the basilar artery were smooth, without thickening, endothelial cells arranged in neat rows, with normal structure, B group was significantly vasoconstriction, endothelial cell degeneration, swelling significantly, C, D groups were slight contraction of blood vessels, endothelial cells slightly swelling. Immunohistochemical staining observed:Bcl-2and Bax positive cells were mainly for the color brown parts, uniform diffuse staining.(1) Bcl-2positive cells observed were as follows:in hippocampal CA1area, in the A group, bcl-2positive cells in the hippocampus had a very weak expression, while B, C, D had a obviously bcl-2expression, in which C, D group the positive cells had more number stronger expression than B group, while the difference between C and D was not obvious (2) bax positive cells observed were as follows:in the sham group A, bax-positive cells in the hippocampus had a very weak expression, in group B, C, D bax expression in hippocampal CA1area were significant. B group compared with C, D groups, the number of positive cells was more and expression was strong. After statistical analysis, pairwise comparison between groups showed that the hippocampus Bax and Bcl-2expression had significant differences in intensity.Conclusion:1, a simple incision damage,didn’t significantly affect the normal activities and neurological function of rabbit and Bcl-2, Bax expres-sion in the hippocampus.2, SAH could affect the normal activities and neurological function of rabbit and increase the expression of Bcl-2and decrease Bax in the hippocampus.3, Fentanyl possibly played a role in brain-protection by enhancing the expression of Bcl-2in brain tissue and inhibiting the expression of Bax in brain tissue to inhibiting apoptosis of neurones.
