Expression and Clinical Significance of Survivin and YAP in Cervical Carcinoma

Objective:To investigate the expression of Survivin and YAP in normal cervical tissue, cervical intraepithelial neoplasia and cervical carcinoma, analyzing the differences and correlation between them, and to research on the relationship between the expression of Survivin and YAP in cervical carcinoma and the development of cervical carcinoma and to explore their clinical significance.Methods:Clinical data collection: Specimens surgically resected of 83 cases of cervical intraepithelial neoplasia and cervical carcinoma were collected from N0.1 Hospital of Dalian Medical University, including 21 cases of cervical intraepithelial neoplasia, 62 cases of invasive cervical carcinoma. Invasive cervical carcinoma include 10 cases of Ia, 31cases of Ib, 17cases of IIa, 4 cases of IIb and 49 cases of squamous cell carcinoma of cervix, 13 cases of adenoid cell carcinoma of cervix. 15 cases of normal cervical tissue were taken as a control group.Immunohistochemical method and results assessment: The expression of Survivin and YAP in normal cervical tissue, cervical intraepithelial neoplasia and cervical carcinoma was detected with the method of immunohistochemistry: PV-6000-G. Survivin positive reaction was defined as clearly brown granules located in cytoplasm, YAP positive reaction was defined as clearly brown granules located in nuclear and/or cytoplasm. For each case, select 5 represented areas in high power visual field. Staining on the percentage of positive cells: 0, positive cells≤5%; 1, positive cells 6~25%; 2, positive cells 26~50%; 3, positive cells 51~75%; 4, positive cells>76%. Staining intensity of positive cells was assessed into four grades: 0, negative; 1, light brown; 2, brown; 3, dark brown. Then according to the product scores the results were divided into four grades: -(0),+(1~4),++(5~8),+++(9~12).Results:The positive rates of Survivin in CIN, cervical carcinoma were significantly higher than that in normal cervical tissue, P<0.01. There was significant difference between CIN and cervical carcinoma, too, P<0.05. The expressive level was gradually increasing from the clinical stage Ia to IIb, and had significant rank correlation, P<0.05. In adenoid cell carcinoma of cervix the positive rate of Survivin is significantly higher than that in squamous cell carcinoma of cervix, P<0.05. There was significant difference in cell differentiation, P<0.05. There was no significant correlation between the expression of Survivin and patients’age, P>0.05.The positive rates of Yap in CIN, cervical carcinoma were significantly higher than that in normal cervical tissue, P<0.05; But there was no statistical difference between the expression of YAP in CIN and that in cervical carcinoma, P>0.05; Besides the expressive level showed an increased trend from the clinical stage Ia to IIb, though without statistical significance, P>0.05. There was no significant correlation of the expression of YAP with patients’age, tissue type and cell differentiation of cervical carcinoma, P>0.05.The expression of YAP and Survivin in cervical carcinoma was positively correlated, r=0.323, P<0.05.Conclusions:The positive rates of Survivin in CIN, cervical carcinoma were significantly higher than that in normal cervical tissue. There was significant difference between CIN and cervical carcinoma, too. The expressive level was gradually increasing from the clinical stage Ia to IIb, and had significant rank correlation. In adenoid cell carcinoma of cervix the positive rate of Survivin is significantly higher than that in squamous cell carcinoma of cervix. It suggests that Survivin maybe involved in the occurrence and development of cervical carcinoma.The positive rates of Yap in CIN, cervical carcinoma were significantly higher than that in normal cervical tissue. But there was no statistical difference between the expression of YAP in CIN and that in cervical carcinoma. There was no significant correlation of the expression of YAP with patients’age, tissue type and cell differentiation of cervical carcinoma. It suggests that YAP may play an important role as a carcinogenic factor during the early occurrence of cervical carcinoma. Detecting the markers of YAP may help in early diagnosis of cervical carcinoma.The expression of Survivin and YAP in cervical carcinoma was positively correlated. YAP may induce the expression of Survivin, which may participate in the occurrence, development and metastasis of cervical carcinoma by inhibiting apoptosis of cervical carcinoma and regulating cellular mitosis.
