Objective: To investigate clinical characteristics and risk factors of early onset type 2 diabetes with non-alcoholic fatty liver disease.Methods: We selected outpatients and hospitalized patients from Jan 2010 to Jan 2011, in the Department of Endocrinology of the Second Affiliated Hospital of Dalian Medical University, with diagnosis of type 2 diabetes, excluding type 1 diabetes, gestational diabetes and mitochondrial gene mutation diabetes, 865 cases (459 men and 406 women). And 306 cases diagnosis age was less than 40 years old and 559 cases diagnosis age was more than 40 years old. According to whether was the fatty liver, the 306 cases were divided into two groups: fatty liver 184 cases and without fatty liver 122 cases. The 559 cases were divided into two groups: fatty liver 253 cases and without fatty liver 306 cases. General informations of patients, family history of diabetes, biochemical and clinical datas were compared. To analysis clinical characteristics and risk factors of early onset type 2 diabetes with non-alcoholic fatty liver disease.Results:1.In the group of early-onset type 2 diabetes with fatty liver disease, The proportion of male patients was higher; The onset age of diabetes under 30 years old had the highest proportion up to 100%(P<0.05).2.Compared to the group of early-onset type 2 diabetes without fatty liver, the group of early-onset type 2 diabetes with fatty liver had higher BMI, waist circumference, hip circumference, WHR, HbAlc, FBG, 2hPG, FINS, FC-P, 2hC-P, TC, TG, apo-B, serum uric acid, HOMA-IR, GGT ; lower HDL-C and shorter duration(P<0.05).3.Compared to the group of late-onset type 2 diabetes with fatty liver, the group of early-onset type 2 diabetes with fatty liver had higher HbA1c, FBG, TG, GGT and lower FC-P, 2hC-P, HOMA-β(P<0.05). 4.By multivariate logistic regression analysis, Onset age, BMI, SUA, TG and HOMA-IR were identified as independent predictors of non-alcoholic fatty liver disease.Conclusion: The onset age of diabetes in the Early-onset type 2 diabetes with fatty liver disease was advanced. The proportion of male patients was higher. Onset age, BMI, SUA, TG and HOMA-IR were identified as independent predictors of non-alcoholic fatty liver disease. Glucose, triglyceride and uric acid level were important for young obesity man. Losing weight, relieving insulin resistance, controlling the level of triglycerides and blood uric acid were benefit for reducing the incidence of NAFLD, protecting the function of isletβcells and delaying the complications of diabetes.
