Clinical Analyses of 235 Cases of Neonatal Vomiting

Objective:Review analysis to study the clinical characteristic, the distribution feature of the etiology and the prognosis situation of neonatal vomitingMethods:The birth status, the clinical characteristics, the etiological diagnosis and clinical prognosis were retrospectively analyzed in 235 newborns complained of vomiting, who were hospitalized in affiliated children’s hospital of Zhejiang University School of Medicine from January 1st2010 to December 31st 2010.Results:In the 235 cases, a male to female ratio of 1.06:1; there were 206 term-infants(87.7%),22 preterm-infants(9.4%); 137 cesarean infants(58.3%); 31 low birth weight infants (birth weight< 2500g)(13.2%),191 normal birth weight infants(2500g≤birth weight≤4000g); 63 breast-feeding infants (26.8%),106 formula-feeding infants.28 infants with feeding nothing(11.9%). There were 163 (69.3%) infants vomiting within 7 days after birth, of which 126 cases occurred within 24 hours, vomitus mainly were foam, yellow or white mucus, milk and blood etc; While 72 (30.7%) cases occurred after 7 days, vomitus were different with milk block, bile and brown liquid. For the causes of neonatal vomiting, there are 186 cases caused by medical diseases (79.1%), in which 105 cases suffered from infectious diseases (including sepsis, infective enteritis, pneumonia and necrotizing enterocolitis); and 49 surgical diseases, mainly with Esophageal atresia, Duodenal atresia,Congenital megacolon and Malrotation of intestine. Finally,198 infants (84.3%) were cured (including those were improved),19 infants (8.1%) against-advise discharge (means didn’t meet the discharge standards), while 18 infants (7.6%) died (most because parents asked to give up),2 of them died in hospital (one with hernia of brain, one with pulmonary hemorrhagia).Conclusion:Multiple neonatal vomiting occur within 7 days, especially within 24 hours after birth, mainly due to medical disease, the general prognosis is well by reasonable treatment; for surgical disease, early diagnosis, prompt surgical treatment is the key to reducing mortality.
