The Change of ABPM in Patients with Early Chronic Kidney Disease

Application of ambulatory blood pressure monitoring for abnormal circadian rhythm in patients with hypertension and target organ damage are many, but for stage 1-2 chronic kidney disease in patients with non-hypertensive abnormal circadian rhythm is no domestic coverage. In Chinese medicine theory Ziwuliuzhu time under the guidance of medical and observed the patients with chronic kidney disease 1-2 of circadian rhythm , the time period in accordance with the twelve Chen grouping , analyzing twelve Chen and non- dippers to change the relationship . This study aimed to early detection of chronic kidney disease circadian rhythm anomalies , further research on this disease circadian rhythm abnormalities occur and owned by the relationship , to guide clinical treatment. Thesis, including literature review and clinical research in two parts. Literature review including ambulatory blood pressure monitoring (ambulatory blood pressure monitory, ABPM), ambulatory blood pressure index system, the mechanism of circadian variability , blood pressure measurement , blood pressure measurement means , ABPM and casual blood pressure compared to the hazards and circadian rhythm abnormalities in Medicine for the understanding and application of circadian rhythms and other content . Clinical research component , observed in 12 patients with chronic kidney disease and 13 cases of normal urine collected ambulatory blood pressure data, analysis of chronic kidney disease (chronic kidney diease, CKD) group and non- CKD group, the incidence of non- dipper blood pressure situation , CKD group and non- CKD group compared ABPM parameters , according to Chinese medicine and Ziwuliuzhu time theory, the time period in accordance with the twelve Chen grouping , analyzing twelve Chen and non- dippers change correspondence. Through these studies , recognizing that through 24hABPM can be dynamically observed CKD1-2 Non characteristic changes in blood pressure in patients with hypertension and found CKD1-2 terms. Patients without hypertension , abnormal circadian rhythm exists , cKD patients daytime heart rate, nighttime heart rate than non- CKD group is high, and the incidence of non- dipper blood pressure increase , combined with Flowing CKD group theory suggested physiologic nocturnal blood pressure is not reduced, and Chou Shi and Yinshi elevated blood pressure in order to use the medicine time medical and Ziwuliuzhu looking theoretical basis for clinical treatment .
