Clinical Study on Metabolism of Glucose and Lipid and SCL-90 of Type 2 Diabetic Patients with Insomnia with Different Qigong Practice

Objection: to observe effect of different Qigong practice on metabolism of glucose and lipid and SCL- 90 in Type 2 Diabetic Patients with insomniaMethod: 90 cases from sleep medicine and endocrinology departments of Guang Anmen Hospital were divided into 3 groups randomly, the combination of Baduanjin-Qigong and relaxation Qigong group, the combination of Liuzijue Qigong and relaxation Qigong group and control group. In the first group, the patients got intervention of both Baduanjin-Qigong and relaxation Qigong and medication; in the second group, they got intervention both of Liuzijue-Qigong and relaxation Qigong based and medication; in control group, they only got medication. All the patients got measurement of fasting blood glucose, 2h postprandial blood glucose, glucosylated hemoglobin, and SCL- 90 and quality of life scale before the treatment, 2 months after the treatment, and 4months after the treatment. All the data was analyzed with SPSS12.0.Result:1. glucose metabolism Intra-group comparison: for glucosylated hemoglobin, no significant difference in group of Baduanjin-Qigong and relaxation Qigong before and after treatment(P>0.05), difference in group of Liuzijue-Qigong and relaxation Qigong before and after treatment(P<0.05); for fast blood glucose, decrease in Baduanjin-Qigong and relaxation Qigong group, no obvious difference in group of Liuzijue-Qigong and relaxation Qigong and control group before and after treatment(P>0.05). for 2h postprandial blood glucose, significant decrease in group of Baduanjin-Qigong and relaxation Qigong and group of Liuzijue-Qigong and relaxation Qigong after the intervention(P>0.05), while no difference in control group(P>0.05).Comparison between group: regulation of fast blood glucose and glucosylated hemoglobin in group of Baduanjin-Qigong and relaxation Qigong with poor control of them is better than that in control group(P<0.05).2. lipid metabolism Intra-group comparison: in group of Baduanjin-Qigong and relaxation Qigong, increase for TC and LDL after the treatment (P<0.05), significant decrease for VLDL and TG (P<0.05) . In group of Liuzijue-Qigong and relaxation Qigong and control group, no significant difference for TC, TG, LDL and VLDL before and after treatment (P>0.05) . For HDL, there was no significant difference in the 3 groups before and after treatment (P>0.05).Comparison between groups: no significant difference for TC, TG, HDL and VLDL in 3 groups after treatments (P>0.05) , while VLDL in the group of Baduanjin-Qigong and relaxation Qigong is high than that in the other two groups (P<0.05) .3. life quality Intra-group comparison: decrease for psychology scoring in group of Liuzijue-Qigong and relaxation Qigong and control group(P<0.05) after the treatment, decrease for society scoring in group of Liuzijue-Qigong and relaxation Qigong(P<0.05) after the treatment. For psychology scoring, no difference in group of Baduanjin-Qigong and relaxation Qigong and control group (P>0.05) . for society scoring, no difference in group of Baduanjin-Qigong and relaxation Qigong (P>0.05) after treatment; For physiology scoring and patient scoring, no difference in the 3 groups after the treatment (P>0.05). Comparison between groups: no difference for the scoring of life quality between the 3 groups (P>0.05) .Comparison between group: no difference in the three groups for the scoring of psychology, physiology, society and patient(P>0.05), but physiology scoring is extremely lower in Qigong group than that in control group(P<0.05).4. SCL-90:GSI, SOM, O-C, DEP, ANX, HOS and PSY of SCL-90 of the intervention groups are significantly higher than ordinary models in China (P<0.05), before Qigong practice treatment. After four months of Qigong practice, the GSI, O-C, DEP, ANX, HOS , PSY of SCL-90 were improved dramatically which have no significant difference to the ordinary models in China (P>0.05) .The I-S is significantly better than the ordinary models in China (P<0.01).Comparison among groups: After four months of Qigong practice, the SOM and PSY of SCL-90 are significantly better in the Liuzijue Qigong group than in the controlled group (P<0.05) .There is no significant change in other factors (P>0.05).Differences among groups: After four months of Qigong practice, There is significant difference between the Baduanjin-Qigong and relaxation Qigong group and the control group (P<0.05) .The SOM and PSY of SCL-90 were significantly improved in both Baduanjin-Qigong group and Liuzijue Qigong group compared with that in control group (P<0.05) .There is no significant change in other factors (P>0.05) .Intra-group comparison: After four months of Qigong practice, the total score, mean score , score of PST, SOM, DEP and ANX of SCL-90 are significantly better in the Baduanjin-Qigong and relaxation Qigong group than in the controll group(P<0.05) .The GSI, mean score ,SOM, I-S, ANX, HOS and PSY are significantly better in the Liuzijue-Qigong and relaxation Qigong group than in the control group(P<0.05) .There is no significant change in other factors (P>0.05) .Conclusion:1. according to present and previous research, TCM Qigong has effect on regulation of fast blood glucose and glucosylated hemoglobin in short term, specially on them in patients of poor control of blood glucose and glucosylated hemoglobin, however, future research should be done.2. The two Qigong programs show no improvement on life quality of type 2 diabetic patients with insomnia in this study.3. The psychological state of all type 2 diabetic patients with insomnia in this research is relatively poor, especially in SOM, O-C, DEP, ANX, HOS and PSY of SCL-90,etc. Qigong practice improves the abnormal psychological state of type 2 diabetic patients with insomnia at certain level.As for the psychological state, Baduanjin combined with relaxation Qigong has better effect in improving SOM and DEP of SCL-90,etc.
