Urinary bladder cancer is the most common malignant tumor, transitional cell carcinoma (TCC) is the most common histological type. Tumor grade, stage is the evaluation of bladder transitional cell carcinoma of the degree of malignancy, the most commonly used predictive prognostic indicators, but due to heterogeneity of tumor biology, grading and staging alone to assess the prognosis has some limitations. Platelet-derived endothelial cell growth factor (PD-ECGF) is an angiogenic factor, most studies suggest that PD-ECGF and bladder tumor grade, tumor recurrence, the prognosis was significantly associated with bladder cancer but its stage, tumor microvessel density ( MVD) related knowledge there are still differences. In this study, 52 cases of bladder transitional cell carcinoma of the paraffin sections using immunohistochemistry to detect the expression of PD-ECGF, coloring microvascular endothelial cells, and a bladder TCC has been confirmed in a strong angiogenic factors - vascular endothelial growth factor (VEGF) as a control, attempts to explore the PD-ECGF, VEGF and bladder TCC clinicopathological features, MVD relationship. Materials and methods Patients and tissue origin: 1994 to 1995, the First Affiliated Hospital, Zhejiang University School of Medicine Department of Urology 52 patients with primary bladder TCC: male 37 cases, female 15 cases, aged 30 to 79 years, mean 62.8 years; TURBt winner of 32 cases, 19 cases of the Ministry of cut, whole cut one case; single tumor in 32 cases, 20 cases of multiple; initial issuance of 35 cases, 17 cases of recurrence. Tumor grading according to WHO criteria: G 1 grade 17 cases, G 2 grade 27 cases, G 3 8 cases. Clinical stage according to UICC criteria: Tis ~ T 1 is 30 cases, T 2 is 18 cases, T 3 2 cases, T 4 for the two cases. 8 cases of normal bladder tissue, six cases of benign prostatic hyperplasia in patients with bladder tissue, 2 cases of non-normal deaths of fresh bladder tissue, all male, aged 32 to 75 years, mean 64.4 years. Follow-up data: Through home visits, combined with a telephone survey methods on all 52 cases selected cases for five years follow-up Master degree thesis. 52 patients, 1 patient was lost, l patients died during follow-up because of its illness, l case full cut not statistics recurrence, 49 cases were successfully followed up. Which recurrence in 28 cases, 21 cases without recurrence (disease-free survival of 42.9%); postoperative death in 10 cases, 39 cases survived five years (overall survival 79.6%). Experimental Methods: Envision rabbit immune histochemical staining methods indulge normal bladder tissue and bladder TCC gaze intently in the PD-ECGF, VEGF expression and MVD distribution. Results found: PD-ECGF, VEGF in the cytoplasm or nucleus appears clearly colored positive sign, the percentage of positive cells staining combined with staining intensity integrated meter divided into qualitative analysis. Percentage of positive cells: unmarked is 0, <25% is denoted 1,26-50% denoted 2,> 50% is denoted by 3; uncolored as 0, yellow denoted by 1, 2 denoted brown, brown Brown denoted 3. Qualitative criteria: two summed score 0-3 divided into negative, positive is divided into 4 to 6. Select microvascular endothelial cell coloring intensive areas (heat island), count five 400 X HPF microvessels, the mean value is per high power field MVD. Statistical Methods: In this study, all data processing were Duron700 microcomputer using SPSS 10.0 software package, which count data correlation statistics using chi-square test, measurement data correlation statistics using non-parametric statistical methods, multivariate regression analysis using COX model is complete. Results a, PD-ECGF in bladder TCC and normal bladder tissues delay: 52 cases of bladder TCC Vision tissue samples PD-ECGF expression positive rate of 50%, 8 cases of normal bladder muscle tissue PD-ECGF expression of all negative . OK Fisher exact test, P (.01, there was a significant difference. Two, PD-ECGF expression and gaze intently TCC bladder pathology, clinical features and prognosis: With the increasing tumor grade, PD-ECGF expression was increased, P <0.01, there was a significant difference; along with increased tumor stage, PD-ECGF positive rate increased, but the line chi-square test P> 0.05, no significant difference, and the other to h-T. and T; a T. divided into two groups, the line chi-square test P <0.05, significant difference; PD-ECGF and tumor single / multiple, early / recurrence were not significantly related, P> 0.05; postoperative recurrence group PD-ECGP The positive expression rate is much higher than the no recurrence group, chi-square test shows K.05, there are significant differences, superficial tumors and postoperative recurrence group PD-ECGF expression was also higher than the no recurrence group, but P two 0.071 and found no statistically significant difference: PD-horses GF and tumor prognosis, no significant correlation, P Chuan .05. 3 Graduate Thesis three, VEGF jump in bladder TCC and normal bladder tissues wrist: 52 cases of bladder TCC tissues balcony positive rate of VEGF sl.9%, 8 cases of normal bladder tissue specimens Vision VEGF expression were negative. OK Fisher exact test, P <0.01, there was a significant difference. Fourth, VEGF expression squint with bladder TCC pathology, clinical features and prognosis: this experiment was not found in VEGF expression and tumor grade and stage. single / multiple, early / recurrence, postoperative recurrence, prognosis, OK chi-square test, all P> 0.05. V. bladder muscle tissue with normal bladder Vision redundant in C WD Distribution: 52 cases of bladder TCC Vision MVD distribution 16.2-75.0 tissue samples (soil 16.425 38.432 ), 8 cases of normal bladder tissue specimens Vision gro distribution 4.4-23 * 4 (11.225 soil 5.961), Mann-whitnev tests show Bu. 001, there was a significant difference for statistical, respectively, 22,37,52 for the node, the MVD in bladder TCC wrist divided into low, medium, high, very high four groups six, MVD Distribution and squint bladder TCC pathology, clinical features and prognosis; different levels of the tumor, stage gender, single / multiple, early / relapse, recurrence / no recurrence, survival / death were grouped its microvessel density (MVD) OK chi-square test, were not found to have significant difference, P) 0.05.
